Yesterday evening a customer contacted us to report that they were unable to post new incidents in any state other than "investigating" as the radio buttons were missing.
After some initial diagnosis, we found this to be related to our recent platform upgrades, and a compatibility issue with one of our Javascript plugins.
To get things operating again we rolled back our upgrades while a fix for this bug was worked on.
This issue has now been fixed, and some additional manual QA tests run by the team to verify everything is working as expected.
We have now rolled our platform upgrades forward again along with this new bug fix.
