Fixed subscribe via Slack on Legacy Theme

We had a bug reported that customers running on our legacy theme were receiving error messages when trying to subscribe via Slack.

This has now been fixed.

This legacy theme will soon be sunset, so we encourage all customers to upgrade to our new theme, which comes with a whole host of improvements such as per-incident pages, full incident history, improved subscriber experience, and a Status API.
